ELIGIBILITY: You Must be an Active or Retired member of the New York State Police Chiefs’ Benevolent Association, residing in New York State and in good standing. A copy of your law enforcement identification (ID) MUST BE SUBMITTED with the order form. Any false information will revoke your privilege to have CBA logo plates. Eligibility will be verified. This form is the only form that will be accepted when accompanied with applicant’s personal check or money order.
